Aside from all 113 digital monsters that you can befriend, Digimon Survive features other collectibles called Perceived Memory which gives out lore tidbits and unique items. These Memories can be found when you pull out Takuma’s Phone and scan the area for a glitch or Distortion; however, not all Distortions indicate a Perceived Memory; they could be an item or even a hidden enemy. Takuma will mention that he feels like he heard a voice in his head if it’s a Perceived Memory. While Takuma’s perception help, this guide will also aid you in finding all Perceived Memory locations in Digimon Survive.

Do note that as long as you have access, you can still go to the locations where the Perceived Memories are. Nevertheless, since this is a game with branching routes — Moral, Wrathful, and Harmonious Path — some areas will eventually be locked out after specific points.
